• In this study, we propose a chiral metasurface based on a slanted lattice, which can simultaneously achieve high CD and high Q-factors. This work adopts a square lattice with elliptical air holes as the metasurface unit. By introducing a single asymmetric parameter to break the structural symmetry, BIC is transformed into qBIC. Subsequently, the introduction of a lattice tilt angle further converts the square lattice into a slanted lattice, breaking mirror symmetry and generating a significant chiral response in the metasurface. • The metasurface employs LiNbO3, a material known for its excellent electro-optic modulation properties. At a wavelength of 1358.97  nm, it excites a qBIC resonance, achieving a circular dichroism value as high as −0.82. Moreover, the study demonstrates broad-range dynamic tuning of the CD by applying an external electric field, highlighting its potential for applications in optical switching devices. • A highly sensitive refractive index sensor is designed based on this structure. Within the simulated refractive index ranges for gases and liquids, the sensor demonstrates sensitivities of 125  nm/RIU and 215  nm/RIU, respectively, indicating excellent sensing performance.
