• HCC cells that originate from HBV-infected hepatocytes express HBV antigens. • HBV-specific TCRs recognise HCC cells with HBV-DNA integration. • Side effects observed after HBV-specific TCR T-cell therapy in patients with primary HBV-HCC were manageable. • Clinical efficacy of adoptive transfer of HBV-specific TCR T cells has been observed in selected patients. • Mechanisms of T cell epitope spreading have been observed following adoptive transfer of HBV-specific TCR T cells.
