• Cathepsin K–heparan sulfate interaction is required for normal bone homeostasis. • Heparan sulfate-binding deficient CtsK is hyperactive under C57BL/6 background, leading to an osteoporotic phenotype. • Heparan sulfate-binding deficient CtsK is hypoactive under 129S1 background, resulting in increased bone mass. • Heparan sulfate likely functions as a buffer to prevent excessive and promote sustained bone resorption.
