• REM/non-REM-related, mixed/non-mixed OSA, and gender define four distinct OSA phenotypes. • OSA phenotypes differ in CPAP pressure, titration grade, and acute sleep improvement. • Male, non-REM-related, mixed OSA subgroup required the highest pressure and most complex titration. • Male, non-REM-related, mixed OSA subgroup showed greatest acute sleep improvement. • PSG provides additional phenotyping information beyond the AHI to inform CPAP titration.
