• Three distinct latent profiles of SI were identified among older adults with CHF: high isolation–family reliant, medium isolation–friend connected, and low isolation–family supported. • Frailty consistently emerged as the most central factor linking physical, psychological, and social domains across SI profiles. • Depressive symptoms and poor family function were key drivers of medium isolation, highlighting subgroup-specific vulnerabilities. • High isolation was strongly associated with older age, multimorbidity, impaired activities of daily living, and lower income. • The combined use of LPA and NA provides a methodological innovation that informs tailored nursing strategies to reduce SI and enhance quality of life.
