• Estradiol only had a significant impact on cerebrovascular health in perimenopausal females. • Younger males seemingly had an upper limit of benefit between estradiol and CVR, as well as ATT. • Testosterone had a positive influence on cerebrovascular health in younger males, but negative consequences in pre- and postmenopausal females. • Poorer cerebrovascular health was associated with higher follicle stimulating hormone for both sexes across the adult lifespan. • Sex hormones are linked to cerebrovascular markers crucial in dementia pathogenesis, in a sex and age-specific manner.
