• High triglycerides drive widespread lipoprotein metabolic derangement. • 1H-NMR reveals lipoprotein changes missed by standard lipid testing. • High TG is linked to TRL remnants, remnant cholesterol, smaller LDL and triglyceride enriched HDL. • Serum NMR detects subclinical systemic inflammation in high-TG patients. • IPE improves lipoprotein and inflammatory profiles beyond TG lowering.
