• HLA haplotypes DR3DQ2 and DR4DQ8 are poorly characterized in LADA. • Data from two epidemiological cohorts provides a sound basis for analysis. • DR3DQ2 or DR4DQ8 in LADA patients associate with a severely diabetic phenotype. • This finding can explain, at least in part, the known heterogeneity of LADA. • DR4DQ8 may exert a stronger phenotypic impact than DR3DQ2.
