• Insufficient pain control after surgery is associated with later chronic pain. • Pain management is complex in older patients as the benefit/risk balance of drugs is modified. • A multidisciplinary team developed a post-operative pain management protocols specifically designed for older patients. • The prescription was adequate in less than 20% of cases, and no patient received 100% of the prescribed medications and monitoring. • Our study highlights the challenges in implementing new postoperative analgesia protocols.
