• Improvement in skin fibrosis with AHSCT versus conventional care persists for several years post-procedure, beyond what was described in existing randomized controlled trials. • This study provides crucial evidence supporting the use of AHSCT as a treatment for SSc in a real-world setting. The findings will contribute to the development of treatment algorithms and clinical guidelines. • This real-world approach underscores the need for further research to investigate the effects of AHSCT on other manifestations of SSc.
