• Targeted delivery of Foxp3 using T-IDLE platform effectively reprograms endogenous T cells into functional, immunosuppressive Treg-like cells. • T-IDLEFoxp3 treatment effectively suppresses pathogenic responses, mitigates tissue inflammation, and prevents detrimental tissue remodelling. • This in vivo cell engineering strategy restores immune homeostasis with high lineage specificity and systemic safety.
