• Positive treponemal and non-treponemal tests identified cases of syphilitic uveitis in the TriNetX database. • The cumulative incidence and prevalence from 2013-2024 were 0.36 and 0.27 per 100,000, respectively. • HIV co-infection occurred in 32.49% and 27.85% had low vision or blindness. • A timely diagnosis and intravenous penicillin can prevent irreversible visual complications.
