• Innateimmune dysregulation drivers multiple myeloma progression by fostering immune tolerance, tumor survival, and resistance. • NK and iNKT cell exhaustion, with MDSC and TAM expansion, creates an immunosuppressive niche driving MM progression. • Targeting innate immunity through NK/iNKT cells, checkpoint, metabolic, and microbiota-based therapies may improve MM.
