In the era of precision medicine, radiotherapy and chemotherapy continue to serve as cornerstone modalities for cancer treatment. However, their management is fraught with challenges. Significant interindividual variability in therapeutic efficacy and adverse effects is observed, attributable to inherent differences among patients. Moreover, conventional tumor monitoring techniques are limited in their capacity to track real - time tumor metabolic dynamics and microenvironmental alterations. Raman spectroscopy, an emerging nondestructive technology, offers a novel approach to the precise management of these therapies by enabling real-time, noninvasive monitoring of tumor biological characteristics and treatment responses, leveraging its high sensitivity (detection limit of surface-enhanced Raman spectroscopy: 10(-9)M), subcellular spatial resolution (<1 mu m) and molecular specificity. This review summarizes the applications of Raman spectroscopy throughout the radiotherapy and chemotherapy management process, covering technological advances in drug resistance mechanism analysis, integration with nanomaterials, tumor imaging, and body fluid detection. It also addresses challenges such as matrix interference and insufficient imaging depth. Furthermore, the review explores its clinical translation prospects, including applications in radiotherapy target delineation, therapeutic drug monitoring and real-time treatment response evaluation.
