• Radiation-induced rectal fibrosis is characterized by ferritin-mediated iron accumulation and VDR downregulation. • Iron accumulation promotes rectal fibroblast activation by suppressing VDR expression. • VDR activation inhibits YAP nuclear translocation and reverses iron-induced fibroblast activation. • Iron chelation with deferoxamine alleviates rectal fibrosis via restoring VDR and inhibiting YAP. • Targeting the iron-VDR-YAP axis represents a novel therapeutic strategy for radiation proctopathy.
