Current European Society of Cardiology (ESC) and European Association for Cardio-Thoracic Surgery (EACTS) guidelines recommend concomitant coronary artery bypass grafting (CABG) in patients undergoing valve surgery with coronary artery stenosis ≥70% and suggest considering CABG for stenoses between 50% and 70%.1 These recommendations are based on observational evidence, and randomized data supporting the prognostic benefit of complete surgical revascularization in patients with valvular heart disease (VHD) and coronary artery disease (CAD) are lacking. Moreover, whether functional lesion assessment should guide the decision to perform concomitant CABG in VHD remains uncertain.
