• Largest longitudinal study to date linking Cd exposure with new-onset diabetes. • Chronic Cd exposure was not associated with risk of new-onset diabetes. • Findings were consistent across observed U-Cd range and prespecified subgroups. • The non-significant association persisted among those with high diabetes risk.
