• Our findings bridge macro-scale brain network abnormalities in OCD with the micro-scale spatial architecture of neurotransmitter systems. • Drug-naïve individuals with OCD show high coupling between macro-scale structural and functional network dysfunctions. • By linking the changes in GMV and ALFF components with neurotransmitter mapping, we propose a molecular susceptibility framework.
