• In a large primary prevention cohort (N = 306,183), lipoprotein(a), remnant cholesterol (RC), and high-sensitivity C-reactive protein (hsCRP) were each independently associated with incident myocardial infarction. • RC demonstrated the strongest association with MI risk among the three biomarkers. • MI risk increased stepwise with cumulative biomarker burden, with up to nearly threefold higher risk when all three biomarkers were elevated. • These associations persisted across key subgroups, including individuals with LDL-C < 100 mg/dL and those on statin therapy. • Combined assessment of these biomarkers modestly improved risk discrimination and reclassification beyond traditional clinical factors, supporting their complementary role in identifying residual MI risk.
