• Breaking from conventional logistics cost research that focuses on economic factors, this study integrates a biomechanical fatigue model into packaging cost analysis to explore the interactions between box size, fatigue, efficiency, and cost, and proposes human-centric cost optimization under maximum fatigue threshold constraints. • We use Tecnomatix® Jack Human Simulation to quantify task duration, wrist muscle load and other parameters for different box sizes, ensuring realism and reproducibility. We also incorporate rest time for fatigue recovery into the cost analysis to achieve a more human-centered evaluation. • We conduct sensitivity analysis of unit packaging cost under different maximum fatigue thresholds for small, medium, and large boxes. All relevant data are derived from Chinese national standards, official data, and market research, ensuring the empirical validity of the results. • This study demonstrates that medium-sized boxes achieve a better balance between cost and fatigue, providing a scientific decision-making basis for enterprises to select packaging with improved occupational safety and efficiency.
