• Risk scores and CAC scoring can miss patients with "non-calcific” plaque • Our patient had CAC of zero and borderline PREVENT risk despite extensive plaque • CCTA with AI-QP detects lipid-rich vulnerable plaque beyond calcium scoring • Highest published LAP (30%) in a patient with sarcoidosis, high Lp(a), and NSTEMI • High Lp(a) plus one risk enhancer may warrant CCTA even when CAC is zero
