• NODDI detects microstructural damage in hippocampal subfields of T2DM patients more sensitively than DTI or volume. • DTI and NODDI consistently show that HATA is a key region for microstructural changes in T2DM. • Increased NDI values may reflect compensatory neuroplasticity in T2DM patients to counteract neuronal damage. • Microstructural damage in hippocampal subfields may contribute to cognitive decline in T2DM patients.
