• Sex-specific multimorbidity patterns in AUD were examined with national registry data. • Females with AUD had greater multimorbidity than males. • Females showed distinct mental-somatic overlap; males showed within-domain patterns. • Pain disorders uniquely bridged mental and somatic conditions in females with AUD. • Findings support sex-specific screening and integrated treatment approaches for AUD.
