• A physics-based model links hepatic fat content to tissue electrical properties. • Fat-dependent electrical properties were validated using liver-mimicking phantoms. • MRI-derived fat fraction maps enabled patient-specific modeling. • Higher hepatic fat content reduces temperature rise and ablation volume in MWA. • The digital twin framework improves personalized MWA planning for fatty liver.
