• This study documents the first large-scale emergence of A. baumannii SToxf2499 in Tianjin, China. • We found an extremely high prevalence of SToxf2499 A. baumannii strains which co-carriaged of blaOXA-23 and blaOXA-66 and exhibited high carbapenem resistance rate. • SToxf2499 rapidly develops multidrug resistance through the formation of IS15D1 composite transposon, thus facilitating its spread within hospital environments. • The IS15D1 composite transposon exhibited a high degree of sequence similarity and coverage with that reported in SToxf191 from South Korea.
