• High myopia in children shows high rates of macular and peripapillary abnormalities. • Axial length (≥26 mm) is a key risk factor, even with moderate refractive error (-6.00D to -0.50D). • Children with high myopia have thinner choroid and peripapillary RNFL, except thicker temporal RNFL. • Longer axial length and thicker temporal RNFL independently predict myopic maculopathy. • Children with -6.00D<SER≤-0.50D but AL≥26mm need clinical attention similar to those with SER ≤ -6.00D.
