• Optimized MRI/MRCP remains the foundation of pancreatobiliary imaging. • EUS adds value selectively when unresolved questions may change management. • CE-EUS, elastography and cyst-fluid molecular analysis expand EUS assessment. • EUS limitations include operator dependence, altered anatomy and tail lesions. • AI-assisted MRI may further narrow selected diagnostic gaps with EUS.
