• EVs mediate antibiotic resistance via target concealment, downregulating porin OmpU and PBP2. • EV-induced metabolic reprogramming toward amino acid scavenging supports survival under carbenicillin stress. • EVs suppress copper efflux (CusRS) and iron acquisition (TonB) under combined antibiotic and copper stress. • EV-driven PTS hyperactivation causes metabolic burnout, increasing bacterial vulnerability to heavy metals. • A molecular framework shows EV-mediated resistance as a double-edged sword in multi-stress environments.
