• Little is known about differences in pediatric and non-pediatric radiologist neuroimaging interpretation in child abuse. • Comparative analysis of 100 cases of pediatric neuroimaging reads revealed 33 in disagreement, with 15 major and 21 minor. • Common major disagreements involved missed skull fractures and missed intracranial hemorrhages. • Consequences of major disagreements included radiation exposure, CPS involvement, and law enforcement involvement. • Accurate neuroradiology interpretations are essential for clinical and investigative aspects of child abuse evaluation.
