• Neuromodulation represents an established therapeutic approach for overactive bladder (OAB), offering both minimally invasive and implantable options. • Sacral neuromodulation (SNM) is the standard treatment for refractory OAB, while tibial nerve stimulation (including implantable devices), a less invasive alternative, is primarily indicated for OAB who do not respond to pharmacological therapy. • Pudendal and peroneal neurostimulation show preliminary promise but are currently not part of routine clinical practice. • The integration of artificial intelligence (AI) into clinical decision-making may improve patient selection for each neuromodulation strategies to enhance therapeutic outcomes.
