• Higher NLR is associated with poorer memory performance at baseline in iRBD. • Higher NLR is associated with worsening of cognitive functions over time in iRBD. • NLR-cognition associations are specific to iRBD and absent in healthy controls. • Peripheral inflammation may contribute to cognitive decline in α-synucleinopathies.
