Localized therapy via oral drug delivery remains particularly challenging for inflammatory bowel diseases (IBD) due to the distinct anatomical and pathophysiological features of the colonic region in the gastrointestinal (GI) tract. Despite extensive exploration of diverse formulation strategies for IBD management, clinical translation has remained limited, resulting in a paucity of effective oral treatment options. This review integrates current mechanistic insights into colon-specific immune pathophysiology with emerging trends in oral formulation development. All of which underscores the urgent need for advanced formulation strategies that integrate multiple targets for IBD treatment.
