• The ocular safety of switching to NNTPs after smoking cessation remains uncertain. • NNTP switchers had higher risk of major vision-impairing eye diseases. • Risk elevations were greatest for diabetic retinopathy and refractive disorders. • Complete nicotine cessation remains the preferred strategy for eye health.
