• Increased evidence supports the persistent platelet activation in APS. • APS platelets display not only proadhesive but also procoagulant (TFpos) phenotype. • Increase of TFpos-platelet-leukocyte aggregates is a marker of platelet activation. • TFpos-platelet/TFpos-platelet -leukocyte aggregates may increase the thrombotic risk. • The two-hit APS model should evolve into a multi-hit model.
