• The presence of overweight and/or obesity are common in youth with T1D (≈30% in this cohort) and are associated with higher weight-adjusted insulin doses, possibly reflecting insulin resistance. • Understanding the patterns that contribute to the growing burden of excess weight among youth with type 1 diabetes is critical for optimizing insulin therapy, guiding preventive interventions, and improving long-term health outcomes. • Further studies are needed to identify potentially reversible obesity risk factors in youth with T1D and guide precision management approaches.
