• Opioid-free anesthesia reduces postoperative nausea and vomiting after cholecystectomy. • Provides better early postoperative pain control and less need for rescue analgesia. • Associated with lower incidence of intraoperative hypotension compared with opioids. • Maintains safety without increasing bradycardia or postoperative opioid consumption.
