• Seven Philippine public-hospital EHR studies were synthesized via the CFIR framework. • Self-efficacy was the sole significant predictor of EHR acceptance (96–98 %). • Inner-setting factors, including leadership, climate, and readiness, drive adoption success. • “User resistance” often reflects software–workflow misfit rather than staff deficits. • Participatory governance and phased rollout sustain EHR use in low-resource sites.
