• TOPAS Monte Carlo simulations were performed for pediatric Hodgkin lymphoma 3D-CRT. • Out-of-field absorbed doses were evaluated for multiple radiosensitive organs and tissues. • Out-of-field organ doses remained below 5% of the prescribed dose, except for the thyroid gland. • The results provide quantitative out-of-field dosimetric data for paediatric Hodgkin lymphoma radiotherapy.
