• Patient-family thermal comfort differences in hospital outdoor spaces were analyzed. • Patients' NPET exceeded family members' by 3.59 °C in winter and 2.23 °C in transition. • CD patients had the highest winter NPET (20.96 °C); OD patients led in transition season (21.79 °C). • Patients with recent surgery/anesthesia had an NPET 2.21 °C higher than those without.
