• MXene (Ti3C2)-enabled paper-based aptasensor for Mammaglobin-A detection. • MAMB1 aptamer–MXene interface enables selective recognition of breast cancer CTCs. • Ti3C2 nanosheets enhance electron transfer kinetics and electrochemical signal response. • Ultrasensitive detection of MDA-MB-231 cells with LOD of ∼4 cells mL−1. • Low-cost disposable platform suitable for point-of-care breast cancer diagnostics.
