• Patient adherence to between-session EX/RP predicted treatment outcome in adults with OCD. • Treatment expectancy and baseline avoidance predicted patient adherence to EX/RP. • Patient adherence fully mediated the effects of both treatment expectancy and baseline avoidance on treatment outcome. • Therapists should prioritize monitoring and reinforcing adherence, increasing treatment expectations, and addressing avoidance to optimize EX/RP outcomes.
