• This review identified 53 randomised oncology pragmatic clinical trials (PCTs). • Most were phase III, multicentric, superiority trials of anticancer interventions. • Surrogate endpoints were the most common category of primary endpoint used. • Further work is required to promote patient-centred endpoints in oncology PCTs.
