Spinal injuries in children are relatively rare but can have significant morbidity and mortality. However, this must be balanced against the increased radiation risk among children. Thorough history and physical examination are necessary to risk stratify best imaging to detect serious injury. For CSI, the 2024 PECARN guidelines provide a clinical guideline on who should receive initial radiography vs CT. While research is sparser for thoracolumbar imaging, the majority of children likely need no imaging or can start with radiography based on mechanism of injury and clinical examination. Further research is needed to further elucidate: (1) if use of the PECARN guidelines will decrease radiation exposure among children and if further delineation by age is needed, and (2) to develop clinical imaging guidelines for thoracolumbar injuries in children.
