Periodic Health Evaluations in the Adaptive Athlete Redefining Medical Clearance

Current existing PHE forms need revision to be more specific in standardizing comprehensive but practical evaluations specific to different adaptive athlete populations. This article provides baseline questions and assessments for performing a PHE on adaptive athletes. Additional history taking and assessments can be done as the HCP becomes familiar with the athlete's medical needs. The PHE should consider the individual athlete's impairment, underlying medical condition, and the physical demands of the sport to determine if an athlete has the physical and mental capacity to safely participate in sport, with or without modifications. It serves as a platform to obtain in-depth medical information, determine the baseline health, and serves as an opportunity to establish trust between the athlete and the HCP. HCPs should acquire adequate knowledge on medical issues associated with commonly seen medical conditions and impairments in adaptive sports, therefore optimizing confidence and quality in conducting PHEs. In the future, it is suggested to standardize PHEs for adaptive athletes and support educational opportunities for HCPs in order to redefine medical clearance for sport participation, thus improving the health of adaptive athletes.
