• Perioperative immunotherapy demonstrates meaningful antitumor activity in muscle-invasive bladder cancer across neoadjuvant, adjuvant, and combined perioperative strategies. • Neoadjuvant immunotherapy-based regimens achieved a pooled pathological complete response rate of 38.92% in 42 prospective studies comprising 6,006 patients. • Neoadjuvant strategies showed encouraging pooled survival outcomes, with overall survival of 82.33% and recurrence-free survival of 77.78%. • Grade 3–4 immune-related adverse events were infrequent in the neoadjuvant setting, supporting an acceptable perioperative safety profile.
