• Plant-derived extracellular vesicles (PDEVs) combat aging via antioxidant, anti-inflammatory, gut microbiota modulation, and pro-regenerative effects. • PDEVs show broad therapeutic potential against neurodegenerative, metabolic, neoplastic, and musculoskeletal age-related diseases. • As natural nanocarriers, PDEVs enable efficient drug loading, targeted delivery, and enhanced efficacy. • Standardization of isolation, characterization, quality control, and storage is critical for clinical translation of PDEVs.
