• The heart-brain connection is underpinned by CV and inflammatory signatures. • 8 biomarkers completely mediated associations between LA strain and CSVD. • TNFRSF11A and IGFBP-4 displayed greatest connectivity to circulating biomarkers. • Future studies are needed to elucidate pathological processes linking AtCM and CSVD.
