Prevalence and factors associated with maternal near-miss at the Regional Hospital Bamenda

Maternal Near Miss (MNM) is a woman who nearly died but survived a complication that occurred during pregnancy, childbirth or within 42 days after childbirth. It assesses and monitors the activities aimed at the prevention of maternal mortality. A 2019 systematic review estimated the global prevalence of MNM at 18.67 per 1000 live births. This study aimed to determine the prevalence of maternal near miss and the associated factors at the Regional Hospital Bamenda. A hospital-based cross-sectional study was conducted for a period of three months at the Regional Hospital Bamenda. Data were collected from women who were admitted during pregnancy, childbirth or within 42 days after the termination of pregnancy using a structured pretested questionnaire. Data collected were analysed using R studio. Descriptive analysis was done followed by bivariate and multivariable logistic regression. Chi-squared test was used to determine the strength of association between variables. Statistical significance was considered at a p value < 0.05 and confidence interval of 95%. Among the 725 live births occurring during the study period, 44 women experienced maternal near-miss events, yielding a maternal near miss prevalence of 60.7 per 1000 live births. Of 252 women enrolled in the study, severe haemorrhage and severe pre-eclampsia were the leading causes of MNM. After multivariable logistic regression analysis, ANC follow-up was independently associated with reduced odds of maternal near-miss (aOR = 0.04, 95% CI: 0.01–0.11), while referral was associated with increased odds of maternal near-miss (aOR = 4.16, 95% CI: 1.85–9.59). There was a high prevalence of maternal near-miss at the Regional Hospital Bamenda. Women with no ANC follow-up and women referred were at risk of having maternal near-miss.
