• Only 60% of phase III GI cancer trials included QoL endpoints (2020–2024). • No trial designated QoL as a primary endpoint despite clinical relevance. • Multi-country trials were ∼7× more likely to include QoL assessment (aOR = 0.15). • QoL inclusion highest in biliary and gastric, lowest in pancreatic cancers. • Findings reveal inequities and call for standardized QoL integration.
