• First quantitative occult blood survey in interventional radiology operating rooms. • Blood contamination was widespread across 142 sites (median: 62.21 μg/cm²). • Operating tables showed the highest contamination (median: 71.19 μg/cm²). • Top 20 hotspot sites contributed 64% of total contamination (Pareto pattern). • Three hypothetical contamination pathways proposed for targeted cleaning.
